Evaluation
An evaluation: one conversation, then a plan in writing
It helps adults who want to understand what is behind their anxiety or their sleep, and adults who need a new prescriber.
You sit with one prescriber, in person or by video if you are in Nevada. The prescriber asks, listens and explains, and the visit ends with a plan you can read at home.

The visit
What the hour looks like
- 01
Your words
Why you are here and what you would like to change.
- 02
Your history
Sleep, health, family, and past care, as much as you wish to share.
- 03
What you take
Prescriptions, supplements and anything over the counter, from the list or bottles you bring.
- 04
Options and a plan
What could help, the risks of each and a written plan before you leave.
Limits
What an evaluation is not
Not therapy
We prescribe and do not provide therapy. We can suggest therapists near you.
Not a promise of a prescription
A medicine is one option. Some visits end with none, and the prescriber says why.
Not a legal evaluation
We do not write reports for courts, employers or benefit claims.
Length and cost
An hour, once
The evaluation takes about 60 minutes. After it, your prescriber sets the pace with you: follow-up visits are shorter, and how far apart they are depends on what you take and how you are doing.
Your fee is given to you in writing before you start. How fees work explains what sets it and what you can ask for.
Questions
Three questions about this visit
Do I need a referral?
No. Some plans ask for one; check yours when we confirm your benefits.
What should I bring to it?
The list or bottles of everything you take, recent lab results if you have them, and your questions.
Can a family member come?
Yes, if you wish. Tell us when you write and we will plan for it.
Ask for a visit, with the times that suit you
We reply within one business day, and your fee is in writing before you start.
